Recommendation Request Form, which can be downloaded by clicking the button in the bottom, to be filled by an applicant who desires of attending a Programme such as higher studies, workshop or short-term course, seminar, conference, etc. with a recommendation of NOC Sri Lanka or whoever is seeking for a recommendation letter from the NOC Sri Lanka.

This form has to be duly filled and should be sent via natolcom@slt.lk at least seven days working days in advance to the required date. For each recommendation letter, the applicant should pay an administration fee of LKR. 1,000/= to NOC Sri Lanka's account: People’s Bank – Town Hall Branch, Account Number: 167-1-001-8-0002036 in favour of National Olympic Committee of Sri Lanka and the payment slip should also be attached with this form.

Please note that the NOC Sri Lanka will not issue any visa facilitating letters. You may request only the recommendation/ certification for the certificates issued by or through NOC Sri Lanka.
